Your First Visit
If you call our dental office directly, you’ll speak with our new patient coordinator, who is happy to look over your dental insurance plan as well as set expectations for your first visit. You’ll be asked to fill out new patient forms at home, then send it over to us online. When you arrive for your appointment, we’ll capture X-rays and introduce you to Dr. Hammond, who performs a detailed oral exam. If further treatment is needed, you’ll have a chance to go over your plan with our treatment coordinator so you can get a better idea of your costs as well as get scheduled for a future visit.
Patient Forms
When you’re at the dentist, it’s safe to assume you’re not exactly interested in filling out a bunch of forms before you can see someone. For that reason, we recommend that you download, print, complete, and bring your forms to the dental office ahead of your appointment.
